Professionals! SWHS consists of two acute-care hospitals: Inland
Valley Medical Center in Wildomar, CA and Rancho Springs Medical
Center in Murrieta, CA. Inland Valley Medical CenterThis campus of
Southwest Healthcare System, a 122-bed acute care hospital, was
recently awarded in the Fall of 2018 with an "A" Grade from the
Leapfrog Group as having the highest level of patient safety as
well as the Top General Hospital Award from Leapfrog (one of only
35 hospitals to earn this distinction nationwide). Inland Valley is
the only hospital in the region to offer Level II Trauma Services,
an Advanced Certified Primary Stroke Center, the region's only
Total Joint Center now with advanced certification from the Joint
Commission for hip and knee surgery as well core certification for
shoulder surgery and is a nationally recognized center for
weight-loss surgery by the Metabolic and Bariatric Surgery
Accreditation and Quality Improvement Program (Also awarded the
Blue Distinction for outstanding bariatric surgical services).
Rancho Springs Medical CenterThis campus of Southwest Healthcare

responsible for identifing and prioritizing the need for further
assessment based on the patient's clinical presentation/diagnosis,
care setting, desire for care and responses to any previous care.
Initiates discharge planning for each patient upon admission as
appropriate. Demonstrates sound judgment and effective decision
making in following the nursing process.Case Managers practice in
accordance with all regulatory, professional, legal and hospital
policies as well as consistently demonstrate behaviors that model
the SWHS Core Values and the hospitals mission statement.Effective
written and verbal communication skills in the English
language.
Ability to define problems, collect data, establish facts, and draw

Qualifications
JOB REQUIREMENTSMinimum three (3) years of acute care clinical
experience.Two (2) years' experience clinical Case Management or
Utilization Review preferred
MINIMUM ESSENTIAL EDUCATIONCompletion of an accredited Registered
Nursing program.
REQUIRED LICENSURE / CERTIFICATIONSCurrent California Registered
Nurse license
